Continuing Care Access

Offers telephone information, screening and referrals for individuals who require access to an Assisted Living program including Home Care, Supportive Living, Continuing Care Home Type A and Type B, Palliative Care, Adult Day Programs / CHOICE, and Children's Home Care.

Provides a telephone number and assistance for people who require access to Assisted Living services in the Edmonton area, including:

  • Home Care (Community and Independent Living Sites)
  • Children's Home Care
  • Day Programs / CHOICE (Comprehensive Home Option of Integrated Care for the Elderly)
  • Alberta Aids to Daily Living (equipment surg supplies)
  • Access to Continuing Care Home Type B and Type B Secure (formerly DSL and DSL4D)
  • Access to Continuing Care Home Type A (formerly LTC)
  • Edmonton Zone Palliative Care Program

Note

After hours support is available from 4:00 PM - 11:45 PM Monday to Friday, and Saturday and Sunday 7:00 AM - 11:45 PM for those individuals already on an Assisted Living Program within the Edmonton Zone & urgent new referrals.

 

Call-backs for new inquiries are made between the hours of 07:00AM and 10:30PM -7 days a week - Please ensure unavailable times are specified during initial call.

Eligibility

Anyone living in Alberta with a valid, active provincial health-care card can be assessed

Service Access

Healthcare providers should consult the Alberta Referral Directory for service referral information.

Consent, Alberta Health Care number and current demographic information is required.

Call-backs are made between the hours of 07:00AM and 10:30PM 7 days a week - Please ensure unavailable times are specified during initial call.

Wait Times

Preliminary information is initially received by administrative support staff. Return call are Triage based on urgency and identified needs.
