Anyone living in Alberta with a valid provincial health-care card (or registration in progress) can be assessed for Continuing Care services. Each client will be individually assessed.

Consent, Alberta Health Care number and current demographic information is required.
Preliminary information is initially received by Administrative Support staff. Calls are then returned based on priority. Calls are typically returned within 7 days, however call-back times can vary depending on call volumes. Call-backs are made between the hours of 07:00AM and 10:30PM - Please ensure unavailable times are specified during initial call.
