Palliative Care Team

 Location: South Zone and Area

Supports individuals and their families during the dying process and at the end of life. Service is offered in a person's home, in hospitals, and in other care centres.

Palliative Care values life and accepts death as a normal process. It does not hasten or slow death. Our team offers support to improve the person's quality of life.

This program includes the person with the disease and the person's family. It focuses on symptom relief as well as emotional and spiritual needs, being mindful of unique cultural practices.

The Palliative Care team helps individuals and their families when there is no known cure for an illness that threatens life. Services include:

  • assessing and dealing with pain and symptoms
  • planning and teaching around the end of life
  • inpatient care on the Palliative Care Unit

Referral Needed

Criteria for palliative care referrals are:

  • Metastatic cancer with unmanaged symptoms or prognosis < 6 months
  • All stage IV lung cancer
  • Other life limiting illness with estimated prognosis <6 months and unmanaged symptoms
  • Consideration for admission to Palliative Care Unit (prognosis <3 months, M2/C1/C2 goals of care)
