Provides education, information, resources, and support to people with breast cancer or suspected breast cancer during initial diagnosis and surgery.Helps patients access services in a timely manner, and minimize delays in receiving care.
Services include:
- care coordination by nurse navigators who are specialized in breast health and cancer
- coordination of diagnostic tests and other appointments, such as mammograms, ultrasounds, and biopsies
- patient education to assist with decisions about treatment options
- support from clinical social workers
- help resolving issues regarding appointment scheduling, as well as problems getting to appointments such as transportation and child care issues
Patients remain in the program until no further surgical treatments are required and transfer of care to a cancer centre is confirmed. Patients are discharged from the program if it is determined they do not have cancer or if they do not need to be monitored further.
The program does not offer routine breast (mammography) screening.
