Continuing Care Access Centre

Provides assessment and refers clients to Continuing Care services and programs within their community.

Continuing Care services and programs in the Central Zone include:

  • Home Care
  • Supportive Living
  • Long-Term Care
  • Community-based service information (e.g. Meals on Wheels)
  • Provides help for people in the hospital to arrange healthcare services they will need when they are discharged (e.g. Home Care)
  • AADL (Alberta Aids to Daily Living) helps Albertans with a long-term disability, chronic illness or terminal illness to maintain their independence at home, in lodges or group homes by providing financial assistance to buy medical equipment and supplies. An assessment by a health care professional determines the equipment and supplies that an Albertan can receive through this program
