Palliative and End of Life Care

 Location: North Zone and Area

Provides symptom management, comfort and guidance to people experiencing a life limiting or terminal illness.

We offer support for managing patient problems, concerns and assessments for palliative care for people with a disease that no longer responds to treatments aimed at a cure.

Our interdisciplinary team works closely with families, other health care professionals and community partners to develop effective approaches to treatment.

Your main health care team provides palliative care, whether you are at home, in a hospital or in a continuing care setting. A member of your care team can refer you to the Palliative Care Team. A referral can also come directly from you or a family member.

Our services include:

  • Providing consultative services and working closely with health care professionals, families or persons living with palliative or end of life conditions (life-limiting disease)
  • Helping provide full care (physical, emotional, mental and spiritual) to the patient and family with a life-limiting disease
  • Assessing eligibility for hospice care

The Continuing Care Access Team has a 24/7 voicemail option and operates:
Monday to Friday 8:15 am to 8:00 pm
Saturday, Sunday and Statutory Holidays from 8:15 am to 4:30 pm.

doctors, nurse practitioners (NPs), occupational therapists, recreational therapists, registered nurses (RNs), social workers, spiritual health practitioners, mental health therapists
