Addiction and Mental Health- Child and Adolescent - Transitions in Care

The Transitions in Care Program is a voluntary program within Central Zone that provides telephone follow-up with youth and / or their guardians after discharging to community from an inpatient program within our portfolio (Step Up Step Down, Unit 39, and PChAD).

During these phone calls assessment screening tool is used evaluate present moment risk, a conversation occurs to see how the youth is doing since their discharge, and to help aid in any resources / referrals that are needed. This is not a crisis service.
