Specialized Geriatric Services

Specialized Geriatric Services is targeted at providing comprehensive geriatric assessments (CGA) and education aimed at frailty, functional decline, cognitive impairment or falls in adults aged 65 and older.

Focuses on providing clinical consultation and education aimed at medical complexity, frailty and chronic disease, functional decline, cognitive impairment, dementia, delirium or falls in adults aged 65 and older

This service does not replace Seniors Mental Health, whose focus is behavioural and psychiatric issues.

Eligibility

This program is for adults age 65 and over who:

  • have a family physician who is able and willing to collaborate in the care and management of clinic patients, and to resume care when patients are discharged from the clinic
  • who are not violent or suicidal
  • who are not involved with Geriatric Psychiatry except when referral is from Geriatric Psychiatry
Referral Needed

A doctor's referral is required. Assessment is done by the SGS team and internal medicine. Call the clinic for specific criteria.
