Geriatric Consult Team

A multidisciplinary home care team that provides geriatric consultation service to Home Care, home bound clients in the Calgary zone.

The Geriatric Consult Team - complete in-home comprehensive assessment for Home Care home bound clients.

Clients with complex medical functional decline, multifactorial or unexplained falls, near falls or symptoms that could lead to falls or a ER visit, medicaiton concerns / issues and cognitive decline with diagnostic challenges.

Access
Service Providers May Include
nurse practitioners (NPs), occupational therapists (OTs), pharmacists, physical therapists (PTs), registered nurses (RNs)
Eligibility

Adults over 65 years old. (Some exceptions made for younger clients)

Healthcare providers should consult the Alberta Referral Directory for service referral information.

Client must be a Home Care client to access this service.
