Neonatal Follow up Clinic

Provides multidisciplinary neurodevelopmental assessments and care of very high risk infants.

This service offers:

  • family centred multidisciplinary neurodevelopmental and neurosensory evaluations
  • guidance, diagnosis, treatment, interim care, and early referral as needed for infants at risk of developing future health and developmental issues because of perinatal / neonatal problems
  • assistance with transition to educational services
  • surveillance / information on early childhood outcomes of newborns born extremely premature or very low birth weight, and children with other high risk conditions

Service Providers May Include
audiologists, dietitians, neonatologists, occupational therapists (OTs), ophthalmologists, orthoptists, pediatricians, physical therapist (PTs), psychologists, registered nurses (RNs), social workers, speech-language pathologists (SLPs)
Eligibility

Criteria include:

  • 28 weeks gestation or under and / or less than 1000 grams at birth
  • hypoxic ischemic encephalopathy (HIE) treated whole body cooling
  • selected complex invasive therapies in the NICU / PICU
  • clinical referral from a doctor or program because of a specific neurodevelopmental concern
