Continuing Care Access (CCA)

Intake for referral to South Zone Home Care, Supportive Living and Long-Term Care. Wait list management for Supportive Living and Long-Term Care.

This service provides a single point for service access and delivery approach that provides Albertans with reasonable, timely and appropriate access to publicly-funded continuing care health services and homecare . It is based on availability and determination of unmet need.

Eligibility

An individual must have valid Alberta Health Care Number in order to qualify for Home Care Services.
