Pediatric Home Care

Provides health care services for children in the Calgary, and consultation to other home care programs in Calgary Zone and southern Alberta.

Works with other healthcare providers, such as your doctor, ambulatory clinics at Alberta Children's Hospital and the Calgary school system to:

  • support family-centred care for the child at home and / or school
  • assess the child's needs in the home and / or school environment
  • teach and support families as they learn to look after their child's medical needs
  • collaborate with ACH clinics and community pediatricians
  • consult and refer within the multidisciplinary team

Note: Homemaking services are not available.

licensed practical nurses (LPNs), occupational therapists (OTs), physical therapists (PTs), registered nurses (RNs), respiratory therapists, social workers ( SW) , speech-language pathologists (SLP)
Eligibility

Program serves children who are eligible for Home Care from birth to 18 years of age.

Service Access

To refer a child from community call Community Care Access - Intake- Calgary Zone

The Transition Services Coordinator at the Alberta Children's Hospital (ACH) aids in a smooth transition from hospital to home. To arrange Home Care for a child in hospital ask the child's nurse to contact the Transition Services Team. When a referral is made, a Case Manager will be assigned to you. Based on your input and the home assessment, the Case Manager will recommend the services that may best meet your child's healthcare needs.
