Programs & Services


CHOICE

Provides services for older adults want to remain living in their own homes. The goal of the program is to keep clients healthy, at home, and out of the hospital system.

Services may include:

  • Medical monitoring and treatment by CHOICE physicians and nurses
  • Medications dispensed by the CHOICE pharmacy
  • Physical and occupational therapy
  • Social Work (and Pastoral Care Services at a Good Samaritan location)
  • Recreation and social activities
  • Personal assistance with daily tasks, as needed
  • Health and wellness education

This service offers several streams at different locations, including streams focused on:

  • Main stream
  • Mental Health
  • Later-stage dementia
  • Restorative care (e.g., shorter-term, goal-focused programming)

Once referred, you will be contacted by an AHS health professional to ask about your goals, determine with you which stream will best meet your needs, and coordinate your first day at the program.

Service Providers May Include
doctors, health care aides, licensed practical nurses (LPNs), occupational therapists (OTs), pharmacists, physiotherapists (PTs), recreation therapists, registered nurses (RNs), social workers
Eligibility

For adults 60 years and older:

  • with health issues who live in their own homes
  • willing to change their family doctor and pharmacist to CHOICE healthcare providers
  • can attend the day centre regularly. Transport may be provided as needed.

Under 60 years:

  • functionally frail, physically disabled, cognitively impaired, or who have challenging behaviours

Referral Needed

Referrals can be made by a Home Care Case Manger or by Contacting Continuing Care Access.

Wait Times

An estimated wait time will be provided at the time of appointment booking.

Fees

There is a program fee for non-health-related services; medicine costs are billed through Alberta Blue Cross or other plans; and clients cover co-pay amounts for medications. $125.76 per month.

Fee reduction can be assessed on a case-by-case basis.
