Rehabilitation and Community Transition Program

Rehabilitation and Community Transition Program (RCTP) is a short-stay restorative care program that supports medically stable adults who do not need an acute-care level of service, but may require additional assessment, recuperation, and therapy before returning to the community.

RCTP is a short-stay program that supports medically stable adults. Clients who do not need acute care level of service but require Physician oversight and 24/7 Nursing support, while actively participating in rehabilitation to improve function to the point where they can manage at home or in another appropriate care setting.

Fees

There is no fee for the RCTP Program. If you are approved for a continuing care home, a daily room charge may apply.
