Rehabilitation and Community Transition Program

 Location: Carewest Sarcee

A short-stay restorative care program that supports medically stable adults who do not need an acute-care level of service, but may require additional assessment, recuperation, and therapy before returning to the community.

This short-stay program supports medically stable clients who do not need acute care level of service, but may require additional assessment, recuperation, and therapy before returning home or moving into another care setting. Clients receive treatment to help improve their level of functioning to the point where they can manage at home or in another appropriate environment.

Referral is made through Community Care Coordinator, Transition Services.

Fees

If a client is assessed and approved for long-term care placement, designated living, or an approved personal care home, the daily accommodation rate applies.

Financial assistance may be available and can be discussed with the Community Care Coordinator.
