cardiologists, dietitians, doctors, gastrointestinal specialists, geriatricians, internal medicine specialists, neurologists, occupational therapists, orthopedic surgeons, pharmacists, physical therapists (PTs), psychiatrists, registered nurses (RNs), social workers, urologists
EligibilityOlder adults with more than one medical problem. Must be referred by a Geriatric Medicine Specialist after a consultation by same.
Service AccessHealthcare providers should consult the Alberta Referral Directory for service referral information.
Community referrals can be made through the Seniors Health One Line Referral.
Referrals can only be made by Geriatric Medicine Specialists (Geriatricians or Geriatric Nurse Practitioners). Most referrals are for hospital inpatients seen by the Geriatric Consult Service, but some Community-based referrals can also be made through the Seniors Health Clinic physicians.
