Provides accessible and timely services to seniors with a variety of mental health needs and assists with expediting hospital discharges, reducing admissions, and preventing illness relapse. Provides short-term follow-up for recently discharged clients of the Rockyview Hospital, Carewest Glenmore Mental Health Recovery and Rehabilitation Unit (R&R), RGH medical-surgical units following Consult Liaison consultation, and transition units at Sarcess / Glenmore Carewest. GMHOT facilitates transition of clients from acute / subacute care to the community with the main goal of promoting and maximizing wellbeing.
Services provided include:
- case management for up to 6-12 months
- medicine monitoring
- community services referrals
- supportive counselling
- family / caregiver support
- consulting referring psychiatrists
