Complex Care Support Team

 Location: North Zone and Area

The Complex Care Support Team serves North Zone Continuing Care clients and their clinical teams, when clients have complex behaviours that influence their care.

The Complex Care Support team supports patients who have been referred to the Continuing Care stream (Home Care, Designated Living or Long Term Care). Clients may be under 65 years and may included people with dementia, early onset dementia, brain injury, developmental disabilities or other conditions that affect cognition. Our services include:

  • comprehensive assessment and recommendations
  • care planning support
  • link to other specialty resources
  • staff education and coaching
  • support for transition between care settings

The Continuing Care Access team has a 24 / 7voicemail option and operates: Monday to Friday 8:15 am to 8:00 pm.


Statutory Holidays open from 8:15 am to 4:30 pm.

Service Providers May Include
clinical nurse educators, occupational therapists (OTs), placement care managers, recreational therapists, registered nurses (RNs), social workers

Clients in the Continuing Care stream (or referred for service in the Continuing Care stream) where behavioral needs cannot be met with a usual Continuing Care level of service.

Referral Needed

Each case / referral is assessed on a case by case basis.
