Children's Home Care

Supports families with children with complex physical medical needs to help them remain safely in their homes and communities.

Based on assessed unmet needs, Children's HC provides case management along with health and developmental services and supports to children / families in their homes or community setting including:

  • transition from hospital to home
  • acute health needs
  • long term health needs
  • end of life support and care

Access
Service Providers May Include
occupational therapists (OTs), physical therapists (PTs), registered nurses (RNs), registered respiratory therapists (RRT), social workers (SW), speech-language pathologists (SLPs), therapist assistants (TA)
Eligibility

Program serves children and youth, from birth to 18 years of age if eligible for Home Care (based on the Coordinated HC Program Regulations).

Referral Needed

Anyone can refer to CHC as long as the family / client is aware and in agreement with same.

Fees

Private insurance will be explored for some services / needs.
