Offers telephone information, screening and referrals for individuals who require access to an Assisted Living program including Home Care, Supportive Living, Continuing Care Home Type A and Type B, Palliative Care, Adult Day Programs / CHOICE, and Children's Home Care.Provides a telephone number and assistance for people who require access to Assisted Living services in the Edmonton area, including:
- Home Care (Community and Independent Living Sites)
- Children's Home Care
- Day Programs / CHOICE (Comprehensive Home Option of Integrated Care for the Elderly)
- Alberta Aids to Daily Living (equipment surg supplies)
- Access to Continuing Care Home Type B and Type B Secure (formerly DSL and DSL4D)
- Access to Continuing Care Home Type A (formerly LTC)
- Edmonton Zone Palliative Care Program

EligibilityAnyone living in Alberta with a valid, active provincial health-care card can be assessed
