Seniors Health Clinic

Offers assessment, diagnoses and development of treatment plans for older adults with complex health issues.

This service provides a team based approach for multidisciplinary, consultative advice on the assessment,diagnosis and development of treatment management plans of adults age 65 or older with issues relating to medical complexity, frailty and chronic disease, functional decline, cognitive impairment, dementia or falls. Referrals for advice on dementia managment challennges, complex chronic disease managment, decline in functional status, delirium, disrptive behavior, fraility, frequency of falls; pre-op evaluation;reveiw of complex medicaition issues

dietitians, geriatricians, neuropsychologists, nurse practitioners (NPs), occupational therapists (OTs), pharmacists, physical therapist (PTs), registered nurses (RNs), social workers

Ages 65 and older..
