Provides support to community and facility programs.Services may include: refer and liaise with appropriate community and government agencies collaborate with other healthcare providers (discharge planning, family conferences) provide counselling and crisis intervention provide information/education on important resources and legislation (e.g., Personal Directives, Guardianship, Trusteeship, Child Welfare and Protection for Persons in Care)
