Seniors Mental Health Program Community Services, Seniors Outreach Nurses

Provides non-urgent comprehensive mental health assessments for Seniors.

Offers services to seniors by ruling out physiological factors, and provide care planning / behavior management strategies to clients in facility and in their homes. We assess for a variety of mental health concerns including:

  • psychosis
  • mood related concerns
  • cognitive impairment
  • anxiety etc.
  • act as a liaison between CCMHBI to assist with admissions and attain psychiatric medication recommendations.
  • act as a liaison between community partners such as homecare, family physicians, CCC’s and other community supports to ensure clients have the appropriate supports in place.

We are typically a one-time consult, although will provide follow up in more complex situations. It is important to note that we do not provide therapy, or medication administration.
