  • Provide a broad range of social and family support services
  • Connect clients with community supports and resources
  • Advocate for clients, perform intervention, provide counseling, referrals, discharge and after-care plans
  • Complete documentation, statistics, reports, and administrative duties associated with assignments
  • Assist in research, evaluation, and policy development projects

Make a Difference

  • Provide emotional support, advocacy, and problem-solving to help resolve relationship issues and improve social well-being
  • Help clients and families ensure basic needs are met, including housing, food, or transportation
  • Provide or help clients access social and family support services, counseling, referrals, discharge, and after-care plans
  • Effect long-term change through policy development, research, and evaluation projects

Work Collaboratively

  • Work with clients and their family members to address issues and solve problems
  • Collaborate with other social workers and inter-disciplinary professionals to deliver patient care
  • Work in a variety of settings, including hospitals, home care centers, and in clients’ homes
  • Connect clients with a variety of external community organizations, programs, and supports

Grow Your Career

  • Move from Social Worker I to II or III and take on clients with more complex, multifaceted issues, lead the implementation and evaluation of services in their area, or lead, supervise, and teach others
  • Additional training and certification
  • Healthcare leadership and management
  • Health system administration
