Health Status Assessment

Population Health Status Assessment refers to the collection and analysis of data and information to profile the health of the population, sub-populations, and communities; and to clarify how the factors that influence health, illness and injury interact and how they can be changed. There is an understanding that many of the factors determining health (commonly referred to as the social determinants of health), lie outside the traditional health sector and include such things as the environment, family and social supports, and economic status.

Population Health Status Assessment staff work to promote the awareness of these determinants and their relationship to health as well as to identify disparities in health so action can be taken by health providers to optimize the health of the whole population.
