Admission and Referral

Patients referrals are accepted provincially from acute care psychiatric and medical inpatient units, community mental health clinics, public and private and family physicians’ offices. The Claresholm Centre for Mental Health & Addictions is a designated facility under the Mental Health Act and accepts formal patients who are in need of rehabilitation.

Admission inquiries and referrals are directed to the Admission Coordinator. Applications are reviewed by an Admission Committee at the Claresholm Centre for Mental Health & Addictions, and a decision about admission is made as soon as possible.

Active Rehabilitation Program

The Active Rehabilitation Program is a 40 bed unit designed for individuals over 18 years of age who are considered able to participate in rehabilitative activities and treatment. The unit provides a structured environment suitable for patients with chronic mental health illnesses, such as psychotic disorders. The goal of this holistic and comprehensive treatment program is to re-integrate individuals back into a community setting within an approximate 3-6 month time frame.

Expected Outcomes:

  • Enhancement or maintenance of adaptive functioning.
  • Meaningful leisure activities based on client interest.
  • Discharge to alternative community placements with appropriate support services.
  • Increased patient and family knowledge of resources available.
  • Outreach support to caregivers and other community agencies for mentally ill patients.

Admission Criteria

  1. Adults (18+ years of age).
  2. Diagnosis: Significant mental illness must be evidenced. An application must be submitted with supporting documents and a referral letter from patient’s psychiatrist is required.
  3. Medical Status: Individuals need to be medically stable and all medical conditions need to be well controlled prior to admission. A recent medical examination is required at the time of referral. Individuals must have sufficient mobility to attend programming which occurs throughout a large facility as well as offsite.
  4. Psychiatric Status: Individuals must be psychiatrically stable with no acute suicidal or homicidal risk. If an individual is diagnosed with a personality disorder(s), their behaviour must be stable enough not to be disruptive and they must be willing and able to tolerate the structure of daily programming.
  5. Skill Development: Individuals must have the potential for skill development which will lead to increased independence. Attendance at all prescribed therapies is required for continued participation in the program.
  6. Therapeutic Environment: An ability to function in an open community-integrated setting is required. Individuals are generally not admitted if they require levels of care that present a risk due to the open therapeutic environment of the Centre. Individuals at high risk for elopement are not admitted to this program.
  7. Pre-Admission Discharge Planning: Individuals requiring identification documents, housing applications, guardianship, trusteeship, and financial assistance should have these applications already made prior to referral so that treatment is focused on patient wellness and reintegration.

Concurrent Disorders Program

The Concurrent Disorders Capable Program services individuals with co-occurring disorder where conditions are sufficiently stable. It is a 6-12 week residential program for individuals 18 years and older. The focus of this program is to provide treatment and education for adults who are coping with a mental health illness and an addiction (substance/behavioral) in order to reach and maintain their overall mental health and well-being. Treatment emphasizes applying recovery skills, preventing relapse, improving emotional functioning and promoting personal responsibility. This is a relatively demanding rehabilitation program. The patient must have been seen and assessed by a Psychiatrist within the past year.

Expected Outcomes:

  • Restoration of adaptive functioning subsequent to a crisis.
  • Enhancement of adaptive functioning within the short term (e.g. time limited) utilizing appropriate interventions.
  • Timely intervention to deal with severe psychiatric disruptive experiences.
  • Comprehensive Assessment Services: the evaluation and identification of specific needs for intervention.
  • Services to provide for and support basic needs of patients at discharge (e.g. economic security, secure stable living environment.)
  • Promotion of community activities which facilitate transfer of information to public at large about mental health issues; supportive networking; service integration/linkages/evaluation.

Target Population

The Concurrent Disorders program provides treatment for individuals experiencing problems with:

  • Anxiety disorders with benzodiazepine abuse
  • Substance abuse with stable mental health disorders including mood disorder and/or anxiety disorder and/or personality disorders which will not prevent effective treatment.
  • This program does not serve individuals who have a Warrant of Detention.

Admission Criteria

  1. The following disorders do not fall within the program mandate of the Concurrent Disorders Program: Schizophrenia, Schizoaffective Disorder, Psychotic Disorder, Substance Induced Psychotic Disorder, Psychotic Disorder Not Otherwise Specified, Amnestic and other Cognitive Disorders.
  2. Adults (18+ years of age).
  3. Detox need: We do not provide acute detox. However, if an individual’s withdrawal needs can be safely managed at the Centre, we can provide a medically supervised slow taper for opioids and benzodiazepines.
  4. Medical status: Individuals need to be medically stable and have the physical capacity to be able to participate and engage in all aspects of the program. Biomedical problems, if any, need to be stable and well controlled prior to admission as evidenced by a recent medical evaluation.
  5. Psychiatric status. Emotional and cognitive functioning must be assessed as sufficiently stable to allow for individuals to fully participate and benefit from the therapeutic interventions provided. Individuals must be psychiatrically stable with no acute suicidal risk, active psychosis or propensity for aggression. If personality disorders are diagnosed, behaviours must be stable enough to not be disruptive and distract from treatment or recovery efforts.
  6. Stage of Change and Willingness: Referrals for admission are preferred to be in the preparation or action stage of change. Individuals must be agreeable to harm reduction/abstinence and motivated for change.
    **For individuals identified to be only in the contemplation or precontemplation stage of change, they must be willing to stop use of all illicit drugs and alcohol and agree to the appropriate use of all prescription and non-prescription drugs including tapering if they are misusing or overusing them.
  7. Relapse Potential: Patients are appropriate if they have a high relapse risk in the community and need a safe residential environment thus making them unsuitable for intensive outpatient treatment.
  8. Recovery Environment: Patients whose home environment is not conducive for recovery may be declined for admission until a structured, supportive environment for discharge is determined.

Extended Rehabilitation Program

The Extended Rehabilitation Program is designed for individuals over 18 years of age, experiencing difficulties in functioning due to ongoing mental health problems. Individuals are provided with rehabilitation support and/or maintenance and an individualized care plan. Individuals on this unit typically require an extended institutional stay (9-12 months or more and are those in need of longer term rehabilitation). Individuals admitted to this unit are usually transitioned into an enhanced supported living environment in the community.

  • Meaningful leisure activities based on patient interest.
  • Discharge to alternative community placements with appropriate support services.
  • Increased patient and family knowledge of resource available.
  • Outreach support to caregivers and other community agencies for mentally ill patients in crisis.

Admission Criteria

  1. Adults (18+ years of age).

  3. Medical Status. Patients need to be medically stable. Their medical conditions need to be well controlled prior to admission. A recent medical examination is required to ensure stability. They must have sufficient mobility to attend programming which occurs throughout a large facility and offsite, as well.
  4. Psychiatric Status: Patients must be psychiatrically stable with no acute suicidal/homicidal risk.
  5. Therapeutic Environment: Patients are appropriate if they have an ability to function in an open and community-integrated setting. Transitions Program

The Transitions Program works within a shared care model whereby patients are managed by the local family physicians with psychiatric consultation, as required. It serves patients with mental health diagnoses who have been assessed and approved by Transitions Services and are awaiting placement in a supported living/long term facility. The unit utilizes a patient tracking security system which can accommodate certain types of patient who have a tendency to wander or elope. Due to environmental and staffing constraints however, the unit cannot accommodate patients who have unprovoked or unanticipated aggressive behaviour towards self or others.

Applications must be completed with supporting documents and include when the patient was assessed by Transition Services, who the Transition Services Worker is and where they are waitlisted for (Transition).

Admission Criteria

  1. Medical Status: Patients must be medically stable
  2. Psychiatric Status: No acting out or physical aggression
