Edmonton Zone
Individuals referred to the Diabetes Program, Edmonton Zone with a new diagnosis of Type 2 diabetes for less than 6 months qualify for education classes only. These individuals need to be monitored by their family physician in the community for 6 months prior to being assessed at a Diabetes Education Centre.
Clients without a family physician may be seen at a Diabetes Education Centre regardless of length of diagnosis.
IGT/IFG referrals require: Fasting/Random glucose
Type 1 require: current A1C and fasting/random glucose. If available include: Lipid panel, albumin/creatinine ratio, creatinine, GFR
Type 2 require: current A1C and fasting/random glucose. If available include: Lipid panel, albumin/creatinine ratio, creatinine, GFR
Pregnancy/Gestational Diabetes referrals require: GDS/OGTT. Type 1 and Type 2 referrals require: current A1C and fasting/random glucose. If available include: Lipid panel, albumin/creatinine ratio, creatinine, GFR
Insulin Pumps: Insulin pump education and assessment sites are limited to the University of Alberta and Grey Nuns Community Hospital. Consultation appointments for insulin pumps are provided for:
a) Individuals currently on insulin and considering insulin pumps
b) Individuals currently using insulin pumps
Please attach a completed Adult Diabetes Program Referral Form.
Identify any special requirements on the referral form: cognitive impairment, hearing / visual impairment, oxygen requirement and/or activity limitations (e.g. walker, cane)
Once a referral is received, the community team will triage the referral to the appropriate service. A Central Access Booking Clerk will contact the client to book for services.
