Alberta Health Services (AHS)

I am an AHS Employee

Your survey code is your Employee ID number.

• If you do not know your Employee ID number, check your ID badge
• Or login to e-People and from the Employee Information page, click on “Employment Details” – “Job Details” and then on the page you will see your number

I am AHS Medical Staff (physician, resident, dentist, etc.)

Your survey code is your CPSA/CDSA/CPPA number.

• If you do not know your CPSA/CDSA/CPPA number, you may look it up:
  • CPSA Physician Directory
  • CPPA College of Podiatric Physicians of Alberta
  • CDSA Dentist Directory - College of Dental Surgeons of Alberta

Residents who do not have a CPSA/CDSA number, use your Employee ID number.

• If you do not know your Employee ID number, check your ID badge
• Or login to e-People and from the Employee Information page, click on “Employment Details” – “Job Details” and then on the page you will see your number

I am an AHS Midwife

Your survey code is your CMA registration number including the leading letters.

• E.g. R1234567 you would enter exactly as R1234567
• If you do not know your registration number, it may be found on your practice permit from the College of Midwives or on the College website.

I am an AHS Volunteer

There is no survey code needed.

Alberta Precision Laboratories (APL)

Select your workforce group. You will be asked for a survey code, see the details below.

I am an APL Employee

Your survey code is your Employee ID number.

• If you do not know your Employee ID number, check your ID badge
• Or login to e-People and from the Employee Information page, click on “Employment Details” – “Job Details” and then on the page you will see your number

I am an APL Physician

Your survey code is your CPSA/CDSA/CPPA number including the leading letters.

• E.g. CPSA123456 you would enter exactly as CPSA123456.
• If you do not know your CPSA number, you may look it up in the Physician Directory - CPSA.

I am an APL Scientific Staff Member

Your survey code is your Employee ID number.

• If you do not know your Employee ID number, check your ID badge
• Or login to e-People and from the Employee Information page, click on “Employment Details” – “Job Details” and then on the page you will see your number

Recovery Alberta

I am a Recovery Alberta Employee

Your survey code is your Employee ID number.

• If you do not know your Employee ID number, check your ID badge
• Or login to e-People and from the Employee Information page, click on “Employment Details” – “Job Details” and then on the page you will see your number

I am a Recovery Alberta Physician

Your survey code is your CPSA/CDSA/CPPA number including the leading letters.

• E.g. CPSA123456 you would enter exactly as CPSA123456
• If you do not know your CPSA number, you may look it up in the Physician Directory - CPSA.
