The Renal ACP (Advance Care Planning) Team helps kidney patients in completing ACP documents such as Personal Directives and Goals of Care Designations. The team also guides patients in their understanding of other legal documents such as Wills and Enduring Powers of Attorney.
The focus is on being prepared for a time of illness or change when someone needs to make decisions for you because you cannot speak for yourself. The team can help you think about your kidney treatment options, what you may want in the future if your health changes and some of the treatment choices your family may need to make on your behalf if you are unable to make these choices yourself. This conversation is personalized to help you deal with the issues that are most important to you and the health challenges you are dealing with.
