Medicine Hat and Lethbridge Home rehabilitation teams accepting new clients

SOUTH ZONE — Alberta Health Services’ Medicine Hat and Lethbridge Home Rehabilitation Teams (HRT) are celebrating their fifth anniversary and want to spread the word about the intensive rehabilitation program available to clients by physician or self-referral.

Unique to the South Zone, HRT is a multidisciplinary team made up of an occupational therapist, physical therapist, recreation therapist, speech-language pathologist, social worker, registered nurse, therapy assistant and dietician who work collaboratively to help clients achieve their goals. HRT clients are individuals who would benefit from involvement with a minimum of two rehab disciplines.

Daily therapy plans are developed collaboratively to help clients meet their own personal goals – from doing laundry, to walking up stairs, to returning to the leisure activities they love. HRT helps clients set realistic and achievable objectives, and provides support and education for patients and their families in their own homes or community environments.

The intensive four to 12-week program provides short-term, client-centered rehabilitation to improve independence, promote shorter hospital stays and prevent the need to move to facility-based living. The program is most suitable for adults with a recent decline in independence, function or mobility living within the Medicine Hat or Lethbridge areas.

South Zone HRT has supported more than 3100 clients since 2017. Space is currently available for eligible clients. Referrals are assessed and prioritized based on need.
