For Your Appointment

Contact your healthcare provider or doctor to ask for a referral to this program. After we receive your intake form, a clinic nurse will review it and call you to set up your first appointment.

What to Expect

The appointment will take about 20 minutes. The nurse will ask questions to learn about your health and needs.

You and the nurse will work together to plan any follow-up care and support you may need. The nurse also works closely with an obstetrician to help make sure you get the right care.

The nurse will also share information about community programs and resources, as needed.
