The Primary Health Care Integration Network’s (PHCIN) main goal is to ensure Alberta’s healthcare system is integrated, so patients experience seamless, coordinated and safe care throughout their wellness journeys.

The PHCIN brings together a variety of partners—including patients, primary care providers, other healthcare professionals, researchers and policy makers—to co-design and implement solutions that address priorities such as improving wait times, streamlining transitions in care, and improving access to specialists.

With strong ties to both primary care and acute care, we are uniquely positioned to foster integration, improve continuity of care and create a more sustainable healthcare system for Albertans.
