Glenrose Rehabilitation Hospital

The Patient and Family Resource Centre provide support, information sessions, and community resources for Glenrose patients, parents, caregivers, and those connected through our community partners.

The family-centered care coordinator is available to support families before, during, and after their Glenrose experience by:

  • preparing parents for their child’s assessment,
  • hosting parent and caregiver information sessions,
  • connecting parents and caregivers to community programs,
  • providing an opportunity for family-to-family connection, and
  • connecting patient advisors with hospital staff to support hospital initiatives and promote collaborative decision-making and inclusive care of patients and their families.

PFAC

Glenrose Patient & Family Advisory Council (PFAC)

A diverse group of patient and family members who have lived experience navigating the Glenrose Rehabilitation Hospital.

Why Join the Council?

Our Patient and Family Advisors will generously volunteer their time to enhance the quality, safety, and effectiveness of health services from the patient, family, and caregiver perspective. They play an important role in supporting the strategic direction of the Glenrose by engaging patients, families and caregivers in a respectful, transparent and accountable manner. Working in partnership with the Glenrose, Council considers existing and emerging topics related to Glenrose programs and services and provides recommendations for consideration.

Being part of the Glenrose Patient & Family Advisory Council is a great way to consult on:

  • Patient, family or staff educational materials creation,
  • New projects involving changes to policies, procedures, programs or services,
  • Capital projects or facility redesign,
  • Patient experience survey design.

Who Can Join?

Members are individuals who have lived experience at the Glenrose, who are passionate about improving care, and they:

  • Engage in co-design and advise Glenrose teams on what we hear and experience,
  • Identify existing and emerging issues,
  • Advise on service delivery, policy and program design, and
  • Provide advice to Glenrose Teams on ways to improve quality, access and sustainability of those receiving care at the Glenrose.

When Does the Council Meet?  

The council holds monthly meetings from September to November and January to June, providing a venue for Glenrose teams to engage directly with advisors and ensure the voices of patients, families and caregivers resonate throughout health policies, projects, and programs across the continuum of care.

CAYAC

Glenrose Child & Youth Advisory Council

A diverse group of children and youth ages 12-21 who have experienced healthcare at the Glenrose Rehabilitation Hospital and are passionate about improving care. As a member, you’ll give feedback on hospital programs, services, and policies, helping to make healthcare better for other youth like you.

Why Join the Council?

  • Being part of the Glenrose Child and Youth Advisory Council is a great way to:
  • Make a difference in how other children and youth experience healthcare at the Glenrose.
  • Meet other youth who share similar experiences.
  • Build leadership skills like teamwork, communication, and problem-solving.
  • Learn about the healthcare system and how it impacts you.
  • Have fun while helping others.

Who Can Join?

Patients and siblings of patients aged 12-18 who have experienced care at the Glenrose are welcome to apply. Once accepted, individuals may stay on the council until age 21.

When Does the Council Meet?

This council will meet in person 10 times per year at the Glenrose. Meetings will take place on Saturdays. Meetings may take up to five hours and include time for breaks and snacks.
