You may request information on who has viewed your electronic health information (audit logs) stored in the Alberta Netcare system by contacting Alberta Health at 1-780-422-5111. More information or to access the form..

To request a copy of who has viewed your electronic health information (audit logs), follow the steps outlined below:

• Fill out the Audit Log Request form.
  • Complete all required areas of the form and sign.  
• Attach a copy of the required identification (ID). Information on acceptable forms of ID. 
• When requesting another individual's health information, include applicable supporting documentation (e.g., Guardianship Order, Power of Attorney, Grant of Administration, etc.). More information regarding authorizations.
  • Note: If supporting documentation is unavailable or applicable authority cannot be identified on the form, clearly indicate the purpose for the request and the processing team will determine if the disclosure is permitted under the Health Information Act.

Audit Log Inquiries

I have received my audit log, and I have questions about who accessed my health information.

If you believe your personal or health information has been improperly collected, used, accessed, lost, or disclosed by an employee or medical staff at Alberta Health Services (AHS), you can submit a breach of information request. More information or to access the Report a Privacy Breach Form.

I have received my audit log, and I have questions about the care that was provided or decisions made by my health care providers.
