Access & Disclosure

Not all personal health information is stored on Netcare. More information or find out what is available on Netcare.

To request a copy of your Netcare records, follow the steps outlined below:

• Fill out the Audit Log Request form.
  • Complete all required areas of the form and sign.  
  • Indicate that you require Netcare records.
• Attach a copy of the required identification (ID). 
  • Information on acceptable forms of ID. 
• When requesting another individual's health information, include applicable supporting documentation (e.g., Guardianship Order, Power of Attorney, Grant of Administration, etc.). 
  • If supportive documentation is unavailable or applicable authority cannot be identified on the form, clearly indicate the purpose for the request and the processing team will determine if the disclosure is permitted under the Health Information Act. More information regarding authorizations.
