ID Badge and Access Card

 

What You Need To Do

You need your employee ID number to request your ID badge and access card.

New Hires:

  1. To request your ID badge, follow the steps below to submit your photo and personal information to your designated zone contact.
  2. After you receive your ID badge, connect with your hiring manager. They will need to request access to the site or department necessary for your role.

Employees Who Have Transferred Positions or Hold Multiple Positions:

  1. Employees transferring positions or sites, or holding multiple positions, must request a new photo ID badge for their new role. A new access card is not needed.
  2. Connect with your hiring manager. They will need to request access to the site or department necessary for your role.

ID Badge Application

The ID badge program ensures security, grants access and identifies staff to patients and their families. All staff are required to display their ID badge while working on site or representing the organization.

If your site has a badging office, you can apply in person with your employee ID number. If you are unable to attend in person, follow the steps below to submit your application online. 


Step 1 – Take a Photo of Yourself

Take a photo of yourself using the guidelines below. Your photo must meet these guidelines!

Do

  • Keep your eyes open, hair back, and use a neutral expression
  • Face the camera straight on with proper exposure
  • Stand in front of a plain background
  • If you wear glasses, ensure your eyes are visible with no glare on the lenses

Don't

  • Tilt your head or camera
  • Stand too far away or in front of a busy background
  • Use direct lighting or have deep shadows
  • Use filters

Step 2 – Save Your Photo

Save your photo as a jpeg using this saving convention: "LastName_FirstName_EmployeeNumber.jpeg".


Step 3 – Submit Your Photo and Personal Information

1. To request your ID badge, submit an email to your designated zone contact:

2. Attach your jpeg photo to the email, and include the following information:

  • Email Subject Line: Your city/town and work site (e.g. Red Deer Regional Hospital)
  • In the email body please provide the following information: 
    • Employee Name
    • Employee ID Number
    • Position Title (e.g. Nurse, Clerk III, Porter, Manager, etc.) 
    • Zone
    • City
    • Site / location (e.g. Foothills Hospital OR 7th Street Plaza, etc.)
    • Department (e.g. Unit 61 OR Finance, etc.)
    • Managers Name
    • Manager Email

Step 4 – Pick Up Your ID Badge

When your ID Badge is ready, you'll receive an email with instructions on where to pick it up — either from your hiring manager, the local badging office, or the site administration office at your work location.


Access Card

After you receive your ID badge, please speak with your manager. They will need to send an email to the designated zone contact to request access to the specific site(s) or department necessary for your role.

To reduce risk, only one access card per employee is provided. Under no circumstances should ID and access cards be shared.
