The following forms are used to determine the amount of tax to be deducted from your employment income or other income, such as pension income. Refer to the Canadian Revenue Agency (CRA) for more information on completing these forms.
Send completed and signed forms by fax at 1-888-908-7265 or to

Note: If you think your net income from all sources will be greater than $165,430, you have the option to calculate a partial claim. To do so, fill in the appropriate section of Form TD1-WS worksheet for the Personal Tax Credits Return and enter the calculated amount on the TD1 form to calculate your basic personal amount.
AHS pays all employees by direct deposit.
On your first day, go the AHS intranet – Insite > Tools > New Employees for instructions on how to add or edit your direct deposit banking information.
To avoid any delays in payment, you must enter your personal banking information within five days of your start date.
AHS employees are paid bi-weekly (every two weeks), on Wednesdays. Your first pay will vary depending on your start date.
Example: Jane’s first day of work was Monday June 2nd (the pay period runs from June 2nd to June 15th). She will receive her first pay on June 25th.
