Connect Care & You

Connect Care is changing the way healthcare is provided in Alberta.

Connect Care changes how patient health information flows between patients and their healthcare providers. With Connect Care, your public health records are accessible from any provincial health agency or corporation in the province. This includes Acute Care Alberta, Alberta Health Services, Assisted Living Alberta, Primary Care Alberta, Recovery Alberta, emergency medical services and Health Link. Your information moves with you through your care journey, giving your provincial healthcare teams a more complete picture of your health history, as well as consistent clinical information and best practices.


MyChart

MyChart is a secure, online, interactive tool that gives you access to your Connect Care health information. The health information viewable in MyChart reflects the Connect Care record that your healthcare team uses. Any contributions that you make to your MyChart is viewable by your healthcare team and vice versa.

You can access MyChart by logging into your MyHealth Alberta Account. Once logged in you will see two tools: My Personal Records and MyChart.

Eligibility

MyChart is available to patients who are aged 14 or older, and who have an Alberta.ca Account. It is also possible to apply for access to health information about another person, typically a young child or family member. This is done through a process called proxy access.
